I don't really ask the question "Are you okay?" to people who don't seem okay - I feel like I'm questioning the obvious. But I guess one of three things could happen - they'll just pretend they're alright and say they're okay, they'll straight up say 'no', or they'll get more pissed and say something like "does it look like I'm okay?" And I'm honestly afraid of hearing that last response for some reason.
